Solution For The Exchange 2010 Event ID 3154 "Active Manager Failed To Mount Database" Error

I've seen the Event ID 3154 error a couple times in Exchange 2010 installations, when IPv6 is disabled on the Mailbox server. Even if you aren't using IPv6, it needs to be enabled or else some weird things can happen. You might notice it on regular mailbox databases or Public Folder databases.

The full error message is:

Active Manager failed to mount database MDB01 on server EXCHMB1. Error: An Active Manager operation failed with a transient error. Please retry the operation. Error: Database action failed with transient error. Error: A transient error occurred during a database operation. Error: MapiExceptionNetworkError: Unable to make admin interface connection to server. (hr=0x80040115, ec=-2147221227).

**Note** The database and server name will be according to your environment of course.

The Fix:

1. On the affected server open the NIC Properties
2. Make sure that IPv6 is enabled.

Mailbox Server NIC with IPv6 Enabled
